Registered Nurse – Community Shift

About the role

As a Registered Nurse in our community shift program, you will provide scheduled in‑home care to patients of all ages with palliative needs. You will help patients stay comfortable at home while fostering therapeutic relationships.

Key responsibilities

• Conduct comprehensive assessments using appropriate tools to determine individual patient needs.
• Develop, implement, and continuously monitor personalized care plans.
• Engage patients collaboratively, building rapport and trust.
• Maintain accurate, up‑to‑date patient records for each encounter.
• Provide on‑call coverage, including after‑hours, to ensure patient safety.

Required profile

• Current registration with the CNO as a Registered Nurse in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient residences.
• Reliability, flexibility, and willingness to travel within a designated area.
• Passion for community care and making a difference.
