Registered Dietitian – Grand River Health Team

About the role

Community Healthcaring KW is seeking a Registered Dietitian to join the Grand River Family Health Team. You will provide evidence‑based nutrition assessment, counseling, and education to individuals and groups, supporting chronic disease prevention and management across multiple community locations.

Key responsibilities

• Conduct individual nutrition assessments and develop client‑focused nutrition plans.
• Lead group education sessions on healthy eating and lifestyle habits.
• Provide counseling and teaching tailored to each client’s needs.
• Apply a Social Determinants of Health perspective to improve health outcomes and reduce inequities.
• Use EMR and other technology to document clinical notes in line with College guidelines.
• Collaborate with physicians, nurses, and other health professionals to deliver coordinated care.
• Participate in community nutrition needs assessments and support food‑security initiatives.
• Work flexible hours, including evenings or weekends, and travel to various Kitchener locations as required.

Required profile

• Current registration with the College of Dietitians of Ontario.
• Valid driver’s licence, reliable personal vehicle, and appropriate insurance.
• Willingness to work both regular and extended hours, including evenings/weekends.
• Strong collaborative skills and ability to work within an interdisciplinary team.

Required skills

• Proficiency with Electronic Medical Record (EMR) systems.
