Registered Dietitian – Diabetes Education (18‑month Fixed‑Term)

About the role

The Diabetes Education Program at Community Healthcaring KW seeks a Registered Dietitian to deliver community‑based nutrition assessments, diabetes self‑management education, and insulin management support for adults with type 2 diabetes in the Kitchener‑Waterloo region. You will work within a client‑centered health team, collaborating with primary‑care practitioners, endocrinologists and community partners.

Key responsibilities

• Provide individual and group nutrition education, health assessments and counseling.
• Develop client‑focused nutritional plans and behavior‑change strategies.
• Utilize EMR and related technology to document and support patient care.
• Collaborate with CHCKW staff, primary‑care providers, endocrinologists and the Waterloo Community Diabetes program.
• Participate in the development, delivery and evaluation of program services.

Required profile

• Registered Dietitian in good standing with the College of Dietitians of Ontario.
• Ability to work within the RD scope of practice as defined by the college.

Required skills

• Proficiency with EMR systems.

What we offer

• Salary range $68,770.86 – $82,857.14 per year (CAD).
• Full‑time (37.5 h/week) 18‑month fixed‑term contract.
