Provider Growth and Relationship Lead

About the role

Amae Health is looking for a driven, relationship‑focused professional to lead referral growth in a defined market. This hybrid position blends territory‑building business development with hands‑on patient advocacy, ensuring a smooth transition from referral to care.

Key responsibilities

• Identify, engage, and manage a target list of referral partners, including hospital systems, emergency departments, discharge planners, and outpatient providers.
• Generate a consistent pipeline of qualified referrals and track performance against growth targets.
• Conduct warm handoffs by meeting patients in hospitals, homes, or community settings to ensure successful onboarding.
• Collaborate with internal clinical, operations, and technology teams to align referral processes with Amae’s care model.
• Travel throughout the assigned territory regularly to build and maintain relationships.

Required profile

• Proven experience in healthcare sales, referral management, or provider partnership development.
• Demonstrated ability to build and sustain a book of business with hospital and outpatient networks.
• Strong communication and interpersonal skills, with a genuine commitment to patient outcomes.
• Willingness to reside within the designated market and travel frequently.
