Part-time Registered Nurse – Fairview Clinic

About the role

VHA Home HealthCare is seeking a part‑time Registered Nurse to join the Fairview Clinic in North York. The clinic operates seven days a week and provides a broad range of home‑health services, from wound care to chemotherapy disconnections. You will deliver client‑centred nursing care while collaborating with a multidisciplinary team.

Key responsibilities

• Assess, monitor and develop individualized care plans for clients referred through Ontario Health at Home.
• Provide direct clinical services including wound care, IV therapy, medication injections, drain care, G‑tube care, blood draws, PICC line removal and chemotherapy disconnections.
• Coordinate care with families, case managers and other health professionals.
• Document all interventions in accordance with College of Nurses of Ontario standards and VHA policies.
• Communicate changes in client condition to the nursing supervisor and case coordinator.
• Participate in continuous quality‑improvement activities to ensure safe, high‑quality care.

Required profile

• Current registration with the College of Nurses of Ontario.
• Experience in community health or home‑health nursing preferred.
• Ability to work independently in a fast‑paced, appointment‑based clinic.
• Strong assessment, care‑planning and documentation skills.
