Outreach Worker – Hepatitis C (Part‑Time)

About the role

The Outreach Worker – Hepatitis C will join the HCV team at Durham Community Health Centre to provide low‑threshold outreach to marginalized individuals living with or at risk of hepatitis C. This part‑time, permanent position focuses on education, support, and facilitating access to testing and treatment across the community.

Key responsibilities

• Conduct outreach activities targeting marginalized groups at risk of hepatitis C.
• Design and deliver education and training sessions on hepatitis C prevention and care.
• Provide direct support services, including counseling and navigation to testing and treatment.
• Promote referrals to appropriate health services and collaborate with interdisciplinary partners.
• Work with community networks and youth‑focused programs to enhance outreach impact.

Required profile

• Experience working with vulnerable or marginalized populations.
• Strong understanding of hepatitis C, its transmission, and treatment pathways.
• Ability to work independently and as part of an interdisciplinary team.
