Nurse Practitioner – Outreach (Part‑Time)

About the role

The Health Centre is seeking a part‑time Nurse Practitioner to join its outreach team serving homeless and precariously housed individuals in Kitchener‑Waterloo. You will provide primary care, health education, and support to mobile teams while addressing complex mental health and addiction needs.

Key responsibilities

• Perform comprehensive assessments, diagnoses, and treatments in line with College of Nurses of Ontario standards.
• Deliver health education and counseling to clients facing social and economic barriers.
• Apply a harm‑reduction approach and work from a Social Determinants of Health perspective.
• Support mobile primary‑care teams and participate in HART hub locations.
• Collaborate with partner agencies within the Inner City Health Alliance.

Required profile

• Registered Nurse (Nurse Practitioner) licence in Ontario.
• Experience working with homeless, precariously housed, or otherwise vulnerable populations.
• Strong understanding of mental health, addiction, and harm‑reduction principles.
• Ability to work effectively within an interdisciplinary team.
