Mental Health Counsellor

About the role

The Mental Health Counsellor will join the Rural Frontenac, Lennox and Addington Allied Health Program, supporting primary‑care practices across the Greater Napanee Health Home. The role involves providing individual, couple, family and group counselling, case management and virtual services to address community mental‑health needs.

Key responsibilities

• Conduct social histories, assessments and therapeutic interventions (individual, couples, families, groups) and provide occasional home visits.
• Facilitate referrals, advocacy and coordination with community health and social services.
• Deliver health‑promotion and education activities, develop resource materials and co‑facilitate group programs.
• Maintain accurate electronic health records, track referrals, and participate in case conferences and quality‑improvement initiatives.

Required profile

• Master of Social Work (or relevant master’s) with active registration in the Ontario College of Social Workers and Social Service Workers.
• Minimum three years recent experience in health/social services, community‑based delivery and brief solution‑focused counselling.

Required skills

• Solution‑focused brief counselling and single‑session therapy.
• Cognitive behavioural therapy and mindfulness techniques.
• Use of Electronic Health Records for documentation.
