Family Practitioner – Home‑Based Primary Care

About the role

The Home‑ViVe Program is expanding its team of physicians to provide 24/7 primary care for frail, homebound elders across Vancouver. You will conduct half‑day sessions, seeing three to six patients per visit, with schedules pre‑booked to reduce travel time.

Key responsibilities

• Deliver comprehensive family medicine services during home visits.
• Maintain a fee‑for‑service billing model comparable to the PMA sessional rate.
• Answer care‑team calls outside scheduled shifts as part of the after‑hours on‑call group.
• Coordinate patient appointments to minimise travel across the city.
• Document encounters using the practice’s electronic medical record system.

Required profile

• Current licence to practice family or general medicine in British Columbia.
• Experience in primary care, preferably with home‑visit or frail‑elder populations.
• Own reliable vehicle and personal cellphone for travel and on‑call duties.
• Willingness to join an after‑hours call group and respond to urgent queries.
