About the role

The Kelowna Senior’s Health & Wellness Centre is looking for a Family Physician to join an interprofessional team dedicated to improving the health and independence of community‑dwelling seniors. The role involves conducting comprehensive geriatric assessments, providing follow‑up care, and collaborating with pharmacists, nurses, and allied health professionals both in‑person and virtually.

Key responsibilities

• Perform comprehensive geriatric assessments for referred patients.
• Conduct follow‑up visits, adjust medications in collaboration with pharmacists and primary care providers, and ensure continuity of care.
• Educate patients, caregivers, and families on diagnosis, treatment, prevention, and self‑management of age‑related conditions.
• Participate in health‑care education initiatives covering continence, falls prevention, cognition, and medication management.
• Coordinate with the patient’s existing community support network and facilitate transitions to community services at discharge.

Required profile

• Licensed Family Physician with a strong interest in senior care.
• Understanding of the unique medical and psychosocial needs of older adults.
• Ability to work effectively within an interdisciplinary team.
• Willingness to provide care both on‑site and remotely.
