Community Health Worker - Outreach

About the role

The Health Centre is seeking a dedicated Community Health Worker to join its Primary Care Outreach team. You will support outreach clinics, including palliative outreach and the Outreach Attachment Clinic, serving individuals experiencing homelessness, housing insecurity, and complex health needs.

Key responsibilities

• Build trust‑based relationships with clients using a Determinants of Health framework and anti‑oppressive, harm‑reduction practices.
• Provide direct client services, coordinate care, and assist with case management and system navigation.
• Develop and support client care plans in collaboration with primary care providers.
• Deliver services in community agency settings and through mobile outreach.
• Maintain accurate, timely records in the Electronic Medical Record system and comply with privacy legislation.

Required profile

• Experience working with vulnerable populations, including those experiencing homelessness or precarious housing.
• Understanding of social determinants of health, harm‑reduction, and anti‑oppressive practice.
• Strong communication and interpersonal skills, with the ability to work collaboratively in an interdisciplinary team.
