Clinical Pharmacist (Part‑Time) – Remote

About the role

HumanisRx is seeking a part‑time Clinical Pharmacist to join its remote team. You will deliver medication‑management services through telehealth programs, collaborating with patients, prescribers and other healthcare providers to optimise therapy and improve health outcomes.

Key responsibilities

• Provide direct patient care as a consulting pharmacist in telehealth programs such as MedMonitor and MedCheckUp.
• Consult with patients, prescribers and other providers about medication therapy, identifying and resolving drug‑related problems.
• Optimize medication regimens to meet individual patient needs while reducing adverse events.
• Contribute to the development, delivery and continuous improvement of medication‑management programs.
• Monitor program quality and effectiveness, and participate in the review of clinical algorithms.
• Escalate clinical issues appropriately and ensure compliance with pharmacy standards, ethics, regulations and privacy laws.

Required profile

• Licensed pharmacist (PharmD or equivalent) authorized to practice in Canada.
• Experience with medication therapy management and remote patient counseling.
• Strong clinical judgment and ability to communicate clearly with patients and healthcare teams.
• Entrepreneurial mindset and willingness to work in a fast‑growing health‑tech environment.

Required skills

• Medication Therapy Management (MTM) expertise.
• Telehealth/remote patient counseling proficiency.
• Knowledge of drug interactions, adverse events and pharmacy regulations.
• Familiarity with clinical algorithm development and review.
