Client Navigator – Primary Care Coordination

About the role

The Client Navigator joins Community Healthcaring KW’s Primary Care team to support individuals with complex social and health needs. You will help clients develop and follow client‑centred care plans, navigate health and social services, and access the resources required for optimal wellbeing.

Key responsibilities

• Assist clients in achieving health goals by guiding them through social service and health‑care systems.
• Facilitate internal and external referrals, including non‑clinical Social Prescribing referrals.
• Support clients in completing disability and benefits forms (ODSP, OCF‑3, CPP, STD/LTD) and coordinate appeals with Legal Services when needed.
• Provide assistance with rental or housing program applications.
• Advocate for clients, develop life‑skill capacities, and work from a determinants‑of‑health perspective.

Required profile

• Experience working with individuals facing multiple health and social challenges.
• Strong understanding of social determinants of health and community resources.
• Excellent communication, problem‑solving, and advocacy abilities.

Required skills

• Knowledge of disability and benefits application processes (e.g., ODSP, OCF‑3, CPP).
• Familiarity with referral coordination and social prescribing concepts.
