CareBridge Nurse Practitioner Manager (Contract)

About the role

The Canadian Mental Health Association, Muskoka‑Parry Sound is hiring a Nurse Practitioner to lead CareBridge, a collaborative primary‑care initiative for residents without a regular provider. The NP Manager will combine direct clinical practice with program leadership, ensuring timely interim care and smooth pathways to long‑term attachment.

Key responsibilities

• Provide assessment, diagnosis, treatment, health promotion and disease prevention as a Nurse Practitioner.
• Coordinate care transitions and manage day‑to‑day clinic operations and team workflows.
• Develop and maintain access and attachment pathways for patients.
• Lead quality‑improvement initiatives and monitor program performance.
• Build collaborative relationships with regional primary‑care providers, the Ontario Health Team and community partners.

Required profile

• Current registration with the College of Nurses of Ontario (Extended Class) and a BScN with Primary Health Care Nurse Practitioner certification.
• Three to five years of Nurse Practitioner experience, preferably in primary care.
• Demonstrated leadership, program‑management or quality‑improvement experience.
• Strong clinical assessment, diagnostic and therapeutic decision‑making skills.
• Commitment to equitable, trauma‑informed, person‑centred and culturally safe care, including Indigenous cultural awareness.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Strong digital literacy and competence with standard office technology.
