About the role

We’re hiring a full-time remote Care Navigator to be the first voice a patient – or their family – hears when reaching out to Sailor Health. In this role, you’ll guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

Key responsibilities

• Guide patients and caregivers through the intake process.
• Match patients with the appropriate therapist and coordinate care steps.
• Provide emotional support and reassurance throughout the journey.

Required profile

• Empathetic and clear communicator.
• Highly organized and detail‑oriented.
• Tech‑comfortable with tools like Google Workspace, Healthie, and HubSpot.
• Mission‑aligned and passionate about improving mental health for older adults.
