Care Coordinator, Health Navigation

About the role

We are looking for a full‑time remote Advocacy Care Navigator to guide Sailor Health patients through our advocacy services. You will support patients who have begun therapy, helping them access additional resources, coordination, and advocacy to thrive.

Key responsibilities

• Conduct introductory calls with patients interested in advocacy services, following a structured intake process.
• Guide patients through the advocacy onboarding journey with warmth, clarity, and attention to detail.
• Coordinate closely with the advocacy team to ensure smooth transitions and follow‑through.
• Manage patient communications across multiple stages, including follow‑ups, missed calls, and reassignments.
• Maintain accurate records using Google Workspace, EHR platforms (e.g., Healthie), and CRMs like HubSpot.

Required profile

• Empathetic and clear communicator who makes patients feel at ease.
• Highly organized and detail‑oriented, able to manage multiple patients simultaneously.
• Comfortable with technology and quick to learn new tools.
• Mission‑aligned with improving care for older adults.

Required skills

• Proficiency with Google Workspace.
• Experience using EHR platforms such as Healthie.
• Familiarity with CRM systems like HubSpot.
