Ambulatory Care Pharmacist

About the role

The Ambulatory Care Pharmacist joins an outpatient, multidisciplinary healthcare team to improve patient outcomes through comprehensive medication management. The role focuses on overcoming access barriers, providing guideline‑directed therapy, and ensuring integrated, accessible health‑care services for the community.

Key responsibilities

• Assess patients’ pharmaceutical needs and deliver appropriate pharmaceutical care.
• Identify and resolve medication access issues and coordinate care when required.
• Build and maintain partnerships with patients, families, and other health‑care providers.
• Stay current with therapy guidelines and advances in ambulatory care pharmacy.
• Counsel patients, families, and health‑care professionals on drug therapy.
• Order, interpret, and monitor laboratory tests as needed.
• Perform medication reconciliations and monitor therapy adherence.
• Detect and prevent drug interactions, contraindications, and adverse reactions.
• Provide preventative care such as vaccinations and wellness screenings.
• Utilise the pharmacy computer system and electronic medical record to document interventions.
• Dispense and administer medications, including immunizations, naloxone, and injectables where permitted.
• Document medication‑therapy‑management interventions and ensure compliance with policies and regulations.
• Support the pharmacy manager or specialty pharmacist and perform additional duties as assigned.

Required profile

• Licensed pharmacist able to work within an outpatient multidisciplinary team.
• Strong commitment to patient‑centered care and medication safety.
• Ability to stay up‑to‑date with clinical guidelines and pharmacy best practices.
• Effective communication skills with patients, families, and health‑care professionals.

Required skills

• Proficiency with pharmacy computer systems.
• Experience using electronic medical records (EMR).
• Knowledge of medication‑therapy‑management processes.
• Capability to perform medication reconciliations and monitor drug therapy.
