Acute Care Nurse Practitioner – Long Term Care

About the role

We are seeking an experienced Acute Care Nurse Practitioner to join our long‑term care team. In this fast‑paced environment you will deliver essential treatment and coordinate care to ensure patients are discharged safely and promptly. The role spans emergency, intensive, and urgent care settings within a hospital environment.

Key responsibilities

• Provide active treatment and necessary care to stabilize patients for discharge.
• Deliver services across emergency rooms, intensive care units, urgent care, and other hospital settings.
• Collaborate with multidisciplinary healthcare teams to manage patient care plans.
• Interpret diagnostic tests and results to guide clinical decisions.
• Maintain high standards of patient safety and quality of care.

Required profile

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
• Current, unrestricted Nurse Practitioner license in the state of practice.
• Certification as an Acute Care Nurse Practitioner (ACNP) or equivalent.
• Minimum of 3 years experience as a Nurse Practitioner in acute care.
• BLS and ACLS certifications.
• Proven ability to manage and coordinate patient care with healthcare teams.
• Strong diagnostic and decision‑making skills.
• Excellent communication and patient education abilities.
