Acute Care Nurse Practitioner – Community Health Care

About the role

Join a fast‑paced community health care team delivering acute inpatient and outpatient services across Canada. As an Acute Care Nurse Practitioner you will provide essential treatment, coordinate patient care, and help patients transition safely from hospital to home.

Key responsibilities

• Deliver acute care in emergency, intensive care, urgent care and other hospital settings.
• Assess, diagnose and manage patients, interpreting diagnostic tests and results.
• Coordinate multidisciplinary care plans to ensure timely discharge of stable patients.
• Educate patients and families on treatment plans and post‑discharge care.
• Maintain high standards of patient safety and quality of care.

Required profile

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
• Current, unrestricted Nurse Practitioner license in the state of practice.
• Certification as an Acute Care Nurse Practitioner (ACNP) or equivalent.
• Minimum of 3 years experience as a Nurse Practitioner in acute care.
• BLS and ACLS certifications.
• Strong diagnostic and decision‑making abilities.
• Excellent communication and patient‑education skills.

Required skills

• BLS (Basic Life Support) certification.
• ACLS (Advanced Cardiovascular Life Support) certification.
• Acute Care Nurse Practitioner certification.
• Diagnostic test interpretation.
