Registered Nurse / Practical Nurse – North Nursing Clinic (Toronto)

About the role

VHA Home HealthCare is seeking a full‑time Registered Nurse (RN) or Registered Practical Nurse (RPN) to join the North Nursing Clinic in the Toronto Central region. The clinic operates in partnership with Ontario Health atHome, delivering specialised community health services such as wound care, IV therapy, catheter care, diabetes and ostomy management.

Key responsibilities

• Assess, monitor and develop care plans for clients in home and clinic settings.
• Provide and coordinate specialised nursing services including wound care, intravenous therapy, catheter care, diabetes management and ostomy care.
• Evaluate interventions through ongoing reassessment and adjust care plans as needed.
• Ensure high‑quality client care and act on factors affecting practice autonomy.
• Maintain professional communication with clients, families and the nursing supervisor.
• Participate in a bi‑weekly weekend rotation and work 12‑hour shifts (8 am‑8 pm).

Required profile

• Current registration with the College of Nurses of Ontario as RN or RPN.
• Experience in community health or home‑care nursing preferred.
• Ability to work flexible 12‑hour shifts and weekend rotations.
• Commitment to client‑centred, evidence‑based care.
