Registered Nurse – Home Care Case Manager

About the role

We are seeking a Registered Nurse to serve as a Case Manager for the Home Care program in the County of Flagstaff, based at the Sedgewick Community Health Centre. You will build relationships with clients and work with a supportive, interdisciplinary team to deliver high‑quality, patient‑centered care in a rural setting.

Key responsibilities

• Assess client needs and develop individualized care plans.
• Coordinate internal and external resources with the interdisciplinary team.
• Provide wound care, palliative care, and home parenteral therapy as required.
• Facilitate client and family education and teaching.
• Ensure timely referrals and modify services on an ongoing basis.
• Provide direct care and be on call as workload dictates.
• Cover additional assignments when needed.

Required profile

• Current Registered Nurse licence (provisional or full practice permit).
• Experience in wound care, palliative care and home parenteral therapy.
• Strong assessment, organizational and leadership abilities.
• Ability to work independently in a rural community and collaborate with a team.
• Willingness to be on call and cover other assignments.

• Part‑time (0.65 FTE) permanent position with union benefits (United Nurses of Alberta).
• Hourly rate $40.76‑$55.79 (CAD) for provisional permit holders, adjusted to full RN rate upon full licensure.
