Pharmacist – Home & Community Care

About the role

The Pharmacy Operations team is seeking a qualified Pharmacist to support daily pharmacy operations for home and community care patients. Working under the supervision of the Pharmacy Operations Manager or Supervisor, you will ensure the safe preparation, verification, and dispensing of both sterile compounded and non‑compound medications.

Key responsibilities

• Perform clinical review of medical orders, confirming appropriateness, cost‑effectiveness, and completeness.
• Prepare, verify, and program infusion supplies and equipment according to patient‑specific orders.
• Conduct visual verification of each step in sterile compounding and perform final checks for particulates and correct labeling.
• Enter prescription orders accurately into the computer system, label, and dispense medications in compliance with Ontario College of Pharmacists regulations.
• Maintain compliance with federal, provincial, and local laws, as well as Bayshore policies, ISO standards, and NAPRA guidelines.
• Collaborate with healthcare providers to provide pharmaceutical expertise and support clinical decisions.

Required profile

• Licensed pharmacist with knowledge of Ontario pharmacy regulations and NAPRA standards.
• Experience in sterile compounding and infusion pharmacy preferred.
• Strong attention to detail and ability to follow strict quality‑control procedures.
