Nurse Team Lead – Neighborhood Health Team

About the role

Bayshore is seeking an experienced registered nurse to lead a Neighborhood Health Team (NHT). You will provide hands‑on client care in the community while mentoring a small group of personal support workers (PSWs). The position offers a balanced schedule with no evenings, nights, or weekends.

Key responsibilities

• Deliver direct nursing assessments and care to clients in their homes.
• Visit clients and caregivers regularly to provide coaching, training, and support.
• Select, train, and mentor PSWs to ensure high‑quality care delivery.
• Build and maintain client relationships and expand the team’s client base.
• Engage in community outreach to develop partnerships and promote home‑care services.

Required profile

• Registered Nurse with solid clinical experience.
• Passion for illness prevention, wellness promotion, and community health.
• Ability to work independently in a self‑managed environment.
• Strong communication and leadership skills to guide a small care team.
