Wellness Nurse

About the role

As a Wellness Nurse at Sunrise Senior Living in Mississauga, you will monitor and promote the health and well‑being of our senior residents. You will work closely with residents, families, and the care team to ensure high‑quality, person‑centered care in a supportive community environment.

Key responsibilities

• Identify and coordinate ongoing health needs of residents, ensuring timely evaluation and documentation of any changes in condition.
• Monitor residents after falls, accidents or incidents, complete required reports, and update the electronic health record.
• Communicate regularly with families and responsible parties about wellness visits, care plans, and any changes in health status.
• Contact attending physicians as needed and assist with medication administration training and oversight.
• Maintain wellness files in compliance with Sunrise policies and provincial regulations.
• Provide clinical support to the community team and participate in quality‑assurance and regulatory‑compliance meetings.
