Home Support Nurse (RN) – Community Health

About the role

Join Vancouver Coastal Health as a Home Support Nurse (RN) in Richmond, BC, providing acute, chronic and palliative care to clients in their homes. Working within an interdisciplinary, team‑based model, you will collaborate with clients, families and caregivers to deliver safe, realistic, and person‑centred care.

Key responsibilities

• Assess client eligibility for home support services and develop individualized care plans.
• Coordinate, implement, and evaluate nursing interventions and community resources.
• Deliver direct nursing care, including wound care, medication administration, and symptom management.
• Provide education and support to clients and families to promote self‑management.
• Maintain documentation in accordance with BCCNM standards and VCH policies.
• Collaborate with other health system partners and incorporate evidence‑based practice.

Required profile

• Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
• At least two years of recent clinical experience relevant to community health or home care.
• Valid British Columbia driver’s licence for local travel.
• Strong knowledge of community health nursing theory, CHNC competencies, and provincial regulations.

Required skills

• Competence in assessing, planning, implementing, and evaluating nursing care.
• Ability to apply evidence‑based clinical practice to diverse client populations.
• Familiarity with acute, chronic and palliative care approaches.
• Effective communication and collaboration within interdisciplinary teams.
