Family Physician for Long‑Term Care Home

About the role

We are looking for a compassionate Family Physician to join a small, collaborative team providing comprehensive medical care to residents of a long‑term care home. The position offers continuity of care in a supportive environment with flexible on‑site scheduling.

Key responsibilities

• Serve as the Most Responsible Physician (MRP) for an assigned panel of up to 30 residents.
• Provide routine, preventive, and acute medical care to long‑term care residents.
• Document all patient interactions using the PointClickCare electronic medical record system.
• Collaborate with two other on‑site physicians and multidisciplinary staff to ensure coordinated care.
• Participate in on‑site visits half a day every other week, with additional remote support as needed.

Required profile

• Board‑certified Family Medicine or General Practice physician.
• Interest in long‑term care and continuity of care.
• Ability to work independently while contributing to a team‑based model.
• Strong communication skills with residents, families, and staff.

Required skills

• Proficiency with PointClickCare electronic medical record.
