Community Hospital Pharmacist

About the role

The Pharmacist will join Brant Community Healthcare System, a two‑site community hospital network serving Brantford and Paris, Ontario. This role supports both inpatient and outpatient pharmacy services, ensuring safe and effective medication management for patients across the system.

Key responsibilities

• Prepare, dispense, and verify medications in accordance with provincial regulations and hospital policies.
• Collaborate with physicians, nurses, and allied health professionals to optimize pharmacotherapy.
• Provide medication counseling and education to patients and caregivers.
• Maintain accurate pharmacy records, inventory, and documentation.
• Participate in medication safety initiatives, formulary management, and quality improvement projects.

Required profile

• Registered Pharmacist with a valid Ontario license.
• Strong knowledge of hospital pharmacy operations and medication safety standards.
• Excellent communication and teamwork abilities.
• Commitment to continuous learning and professional development.
