Registered Practical Nurse – Acute Care (MH)

About the role

The Registered Practical Nurse (RPN) works in a dynamic acute‑care environment as part of an inter‑professional team. The role focuses on delivering safe, effective, and ethical nursing care while promoting patient wellness and supporting the mission of Unity Health Toronto.

Key responsibilities

• Provide holistic, evidence‑based, culturally appropriate patient‑centred care in line with College of Nurses of Ontario standards.
• Establish therapeutic relationships and deliver autonomous care for stable patients with predictable outcomes.
• Collaborate with Registered Nurses and other team members for patients whose condition becomes unstable, ensuring safe transfer of care when needed.
• Assess patient needs, develop and adjust care plans, and document all observations and interventions promptly.
• Educate patients and families using effective teaching strategies tailored to their learning needs.
• Coordinate and communicate care activities to optimise patient experience and outcomes.
• Contribute to discharge planning, transition of care, and advocacy for appropriate care settings.

Required profile

• Current registration with the College of Nurses of Ontario as a Registered Practical Nurse.
• Experience in acute‑care or mental health settings is preferred.
• Ability to assess, prioritize, and respond to rapidly changing patient conditions.
• Strong communication and teamwork skills to collaborate with diverse health‑care professionals.
• Commitment to ongoing professional development and reflective practice.
