Registered Nurse – Primary Care Outreach (18‑month Contract)

About the role

The Primary Care Outreach to Seniors program at SEOCHC is seeking an experienced Registered Nurse to provide high‑quality, home‑based care for frail seniors across the Champlain Region, using a palliative‑care approach.

Key responsibilities

• Conduct comprehensive geriatric assessments and design individualized care plans.
• Provide clinical assessment, screening, illness prevention, and disease management for seniors.
• Address mental health and substance‑use concerns in senior clients.
• Facilitate advanced care planning and serious‑illness conversations.
• Educate clients and promote health among seniors.
• Identify and connect seniors with appropriate community resources.
• Work independently and collaboratively within an interprofessional team.
• Deliver services in community‑based settings, including home visits.

Required profile

• Registered with the College of Nurses of Ontario, without practice restrictions.
• Current Basic Life Support (BLS) certification or equivalent CPR/AED Level C.
• Valid driver’s licence, insurance and personal vehicle for travel.
• Fluency in English and French.
• Proficiency with computers, software applications and electronic documentation.
• Vulnerable sector screening completed within the past 12 months.
• Gerontological nursing certification or training is an asset.

Required skills

• Basic Life Support (BLS) certification.
• CPR/AED Level C certification.
• Computer and electronic documentation proficiency.
