Registered Nurse – Community Home Care

About the role

Join a leading home‑care provider as a Registered Nurse delivering scheduled, in‑home visits to patients of all ages. You will assess individual needs, implement care plans and help patients stay comfortable in their homes for as long as possible.

Key responsibilities

• Use appropriate assessment tools and techniques to evaluate each patient’s unique needs.
• Develop, execute, and continuously monitor personalized care plans.
• Build rapport and trust with patients through a collaborative, compassionate approach.
• Maintain accurate, up‑to‑date professional records of all patient encounters and care plans.

Required profile

• Active registration with the College of Nurses of Ontario (CNO) in good standing.
• Reliability, flexibility and willingness to travel within a designated area.
• Passion for making a difference in the community and ability to share on‑call coverage, including after‑hours.
• Experience delivering community or home‑care services is an asset.
