Part‑time Pharmacist – Community Hospital

About the role

The Brant Community Healthcare System is seeking a part‑time pharmacist to provide clinical pharmacy coverage across its two hospital sites in Brantford and Paris, Ontario. You will join a multidisciplinary team dedicated to delivering high‑quality, patient‑focused care.

Key responsibilities

• Provide medication dispensing, verification, and counseling in acute‑care and urgent‑care settings.
• Collaborate with physicians, nurses, and allied health professionals to optimise drug therapy.
• Participate in clinical rounds and contribute to medication safety initiatives.
• Maintain accurate pharmacy records and ensure compliance with regulatory standards.
• Support transitional care services and assist with medication reconciliation at discharge.

Required profile

• Licensed pharmacist in Ontario with active registration.
• Strong knowledge of hospital pharmacy operations and clinical pharmacy practice.
• Excellent communication and teamwork skills.
• Ability to work independently within a part‑time schedule.
