Part‑Time Family Physician (0.50 FTE) – Primary Care

About the role

Woolwich Community Health Centre is seeking a part‑time Family Physician (0.50 FTE) to provide day and evening primary‑care coverage at its New Hamburg site in St. Jacobs. The physician will join an interdisciplinary team delivering comprehensive health services to seniors, families, youth and rural residents.

Key responsibilities

• Provide acute, preventive and chronic care to a diverse patient panel.
• Conduct injury assessments, diagnose and manage chronic diseases.
• Offer continuous, long‑term primary‑care and patient advocacy.
• Participate in one evening per week and share on‑call duties with a rotating schedule of physicians and nurse practitioners.
• Collaborate with nurses, social workers and other team members to coordinate referrals to community resources.

Required profile

• Current registration with the College of Physicians and Surgeons of Ontario.
• Medical degree (M.D.) and ability to work in a team‑oriented environment.
• 2–3 years of community‑based primary‑care experience preferred; new graduates are encouraged to apply.
• Familiarity with health issues affecting seniors, rural populations, youth and families with young children.
• Capacity to manage multiple and complex demands.

Required skills

• Experience with the PS Suite electronic medical record (EMR) system (preferred).
