Nurse Case Manager

About the role

Cencora is seeking a Nurse Case Manager to lead complex care coordination initiatives that ensure timely patient access, treatment progression, and continuity of care. You will work closely with providers, pharmacies, payers and internal teams to remove barriers and improve patient outcomes.

Key responsibilities

• Plan, manage, and monitor complex care coordination cases from intake through discharge.
• Conduct comprehensive assessments, develop care plans, and facilitate transitions of care addressing clinical, social, and access challenges.
• Collaborate with providers, pharmacies, payers and internal partners to resolve access issues and maintain case continuity.
• Analyze case trends and workflow patterns to identify root causes and recommend process improvements.
• Create and refine job aids, care‑coordination templates and documentation standards to increase consistency and reduce rework.
• Mentor less‑experienced team members on complex case handling, escalation procedures and documentation expectations.

Required profile

• Bachelor’s degree in nursing, social work, health sciences, public health, healthcare administration or a related field, or equivalent experience.
• Minimum 4 years of experience in care management, case management, clinical support or patient services.
• Certification in case management, care coordination or a clinical specialty is preferred.
