About the role
KCHC offers a sustainable primary‑care environment where physicians work within an interdisciplinary team, receiving dedicated time for documentation, care coordination and complex care planning. The role enables full‑scope family medicine practice while maintaining professional sustainability.
Key responsibilities
- Provide comprehensive primary care across the lifespan, including acute and chronic disease management, preventive care and health education.
- Collaborate with an interdisciplinary team of nurse practitioners, social workers, dietitians and other allied health professionals to coordinate care and address social determinants of health.
- Participate in on‑call duties, case consultations, and shared‑care arrangements in accordance with ministry and group guidelines.
Required profile
- Certification in Family Medicine (CCFP) and registration with the College of Physicians and Surgeons of Ontario in good standing.
- Current CMPA (or equivalent) malpractice insurance and satisfactory criminal and vulnerable persons check.
Required skills
- Proficiency with Microsoft Office applications.
- Experience using an electronic medical record system, preferably PS Suite.
