Home and Community Care Nurse – Full‑Time

About the role

The Lillooet Tribal Council is seeking a full‑time Home and Community Care Nurse to join the St’át’imc Outreach Health Services Program. The nurse will work directly with Indigenous clients and families, providing culturally safe care, guiding Community Home Support Workers, and developing wellness plans that empower clients.

Key responsibilities

• Establish trusting, culturally safe communication with clients and families.
• Provide direct nursing care and conduct comprehensive client assessments, including medical history, medications, and support needs.
• Develop and implement individualized wellness plans and support case‑management needs.
• Coordinate referrals, discharge planning, and liaise with external agencies and service providers.
• Deliver health education, workshops, presentations and prepare reports on program status and community health trends.

Required profile

• Minimum five years of relevant nursing experience.
• BSN and registration with the British Columbia College of Nurses and Midwives (RN); LPNs considered based on experience.
• Knowledge of community resources and cultural competency when working with St’át’imc clients.
• Successful criminal record check and a valid driver’s licence.
