Field Case Manager – Nurse (Remote/Travel)

About the role

The Neurology Nurse Field Case Manager supports patients, physicians and health‑care teams across Quebec by coordinating therapy, education and reimbursement processes. Working primarily from home, the role involves regular travel within the assigned territory and occasional overnight stays.

Key responsibilities

• Manage a caseload of patients and physician/clinic contacts, ensuring seamless enrollment and ongoing therapy support.
• Act as the primary point of contact for patients, physicians, health‑care professionals and vendors.
• Provide education, training and adherence monitoring to patients and providers, both in‑clinic and via telephone.
• Coordinate with the reimbursement team to secure drug coverage and report status updates.
• Document all patient interactions thoroughly to maintain continuity of care.
• Report adverse events (AE) and product technical complaints (PTC) promptly.
• Attend scheduled training sessions and complete self‑learning assignments in line with SDM Specialty Health Network SOPs and College of Nurses standards.

Required profile

• Registered Nurse with experience in case management or patient support.
• Strong attention to detail and ability to maintain accurate documentation.
• Excellent communication skills for interaction with patients, clinicians and internal teams.
• Willingness to travel up to 75% of the time within the territory, including occasional overnight stays.
• Ability to work independently from home while meeting business objectives.
