Family Health Educator

About the role

The Family Health Educator works autonomously within a multidisciplinary team to promote optimal family and childhood health outcomes, focusing on early and school years. The role involves counseling, education, case management and system navigation for diverse families.

Key responsibilities

• Provide one‑on‑one and group parenting support, including health education, behaviour management and trauma‑informed care.
• Conduct intake assessments, referrals, case management and follow‑up, and liaise with community agencies and partner services.
• Deliver evidence‑based parenting groups, home visits and outreach, and mentor volunteers or students.
• Contribute to quality‑improvement initiatives and develop system‑based efficiencies to enhance client experience.

Required profile

• University degree in Social Work, Occupational Therapy or Nursing and registration in good standing; Registered Nurse preferred.
• Three to five years of experience in primary care or related community, hospital or public‑health settings.
• Experience supporting vulnerable and diverse families, including those with addictions, mental health or complex psychosocial issues.

Required skills

• Proficiency with Microsoft Office applications.
• Experience using electronic medical record systems, preferably PS Suite.
