Community Wellness & Outreach Worker

About the role

The Community Wellness & Outreach Worker will serve Cold Lake First Nations members, building trusting relationships and connecting individuals and families with wellness programs, cultural supports and essential services.

Key responsibilities

• Build positive, trusting relationships with Cold Lake First Nations members.
• Maintain a visible presence in the community and conduct proactive outreach.
• Connect individuals and families to Horelyu Dene Ts'edi Kue programs, community resources and external services.
• Provide information, referrals, advocacy and follow‑up to address barriers to service access.
• Link members with Elders, Knowledge Keepers and culturally appropriate wellness resources.
• Identify emerging community needs and service gaps and report them to the team.
• Support prevention‑focused programs, workshops, groups and community events.

Required profile

• Approachable, able to build genuine relationships and trust.
• Knowledge of First Nations cultures, values and the impacts of colonization and intergenerational trauma.
• Understanding of trauma‑informed, strengths‑based and culturally safe approaches.
• Strong listening, communication and problem‑solving abilities.
• Ability to work independently in the community while collaborating with team members and partners.
