Clinical Pharmacy Specialist – Medication Safety

About the role

This full‑time relief position supports medication safety at Vancouver General Hospital until September 2027. The Clinical Pharmacy Specialist will apply advanced pharmaceutical knowledge to improve patient outcomes and support the pharmacy team.

Key responsibilities

• Consult with internal and external health‑care professionals on complex drug therapy decisions.
• Provide evidence‑based clinical direction for challenging cases and review patient medication histories.
• Participate in patient education, discharge counseling, and development of drug‑therapy guidelines.
• Identify and initiate clinical drug research projects and prepare case studies for professional journals.
• Deliver pharmacology education sessions and represent the department on multidisciplinary committees.

Required profile

• Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
• Completion of an accredited Hospital Pharmacy Residency Program.
• Doctor of Pharmacy (Pharm.D.) degree.
• At least one year of pharmacy experience in the designated specialty area.
