Clinical Nurse (RN) – Part‑Time Primary Care

About the role

Woolwich Community Health Centre (WCHC) is seeking a part‑time Clinical Nurse (RN) to provide day and evening coverage across its St. Jacobs, Wellesley, and Linwood sites. The RN will join an inter‑disciplinary team delivering essential nursing services to support client care.

Key responsibilities

• Provide comprehensive, evidence‑based primary care services, including health promotion, disease prevention, chronic disease management, and episodic care.
• Coordinate care and support patients through education, assessment, and clinical reasoning.
• Prescribe medications within the regulated scope for qualified RNs, improving timely patient‑centered care.
• Collaborate with physicians, nurse practitioners, allied health professionals, and community partners to ensure culturally safe and equitable care.
• Maintain accurate documentation and manage information using electronic medical records.

Required profile

• Current registration as a Registered Nurse in good standing with the College of Nurses of Ontario.
• 2–3 years of clinical nursing experience, preferably in primary care, community health, family practice, or chronic disease management.
• Demonstrated assessment, clinical reasoning, and patient education abilities.
• Experience working within an interdisciplinary team environment.
• Proficiency with electronic medical records.
