After Hours Pharmacist

About the role

CareRx is seeking an After Hours Pharmacist for its Oakville location. Working under the Designated Pharmacy Manager, you will ensure prescriptions are dispensed accurately and safely while providing patient‑focused pharmaceutical care during after‑hours periods.

Key responsibilities

• Collaborate with the pharmacy team to dispense medication orders accurately, safely, and timely.
• Receive verbal prescription orders and transcribe them in compliance with local regulations.
• Verify safe and appropriate medication administration, including critical and palliative‑care prescriptions.
• Lead and support the after‑hours pharmacy staff in daily tasks.
• Arrange drug deliveries with the delivery team and maintain communication with resident‑care teams.
• Conduct medication regimen and profile reviews.
• Provide exceptional customer care to patients, partner pharmacies, and home health partners.

Required profile

• Licensed pharmacist eligible to practice in Ontario.
• Ability to work independently during after‑hours shifts.
• Strong attention to detail and commitment to therapeutic appropriateness.
• Effective communication skills with multidisciplinary care teams.
