Summer Student – Chronic Care Program Planner

About the role

This summer student position supports the Chronic Disease Education Program by conducting literature reviews, organizing findings, and assisting in the development of clinical decision tools for chronic disease management. The role contributes to building a robust framework for team‑based primary care of conditions such as diabetes, COPD, and heart failure.

Key responsibilities

• Perform systematic literature searches and synthesize evidence related to chronic disease care.
• Organize and present research findings to support program planning.
• Assist in designing and testing clinical decision tools for inter‑professional use.
• Develop an understanding of the Health Professions Act regulations and the Chronic Care Model.
• Create and maintain project timelines, ensuring milestones are met.
• Participate in team and working‑group meetings, providing updates and insights.

Required profile

• Minimum completion of the third year of a Bachelor’s program in Health Sciences, Nursing, or Health Project Management.
• Valid Class 5 driver’s licence (or equivalent) for the province of residence.
• Strong organizational abilities and capacity to work independently.
• Problem‑solving, decision‑making, and flexibility in a changing environment.
• Excellent communication skills and respect for confidentiality and cultural diversity.

Required skills

• Proficiency with Microsoft Outlook, Word, Excel, Teams, and PowerPoint.
• Competence using internet applications and other information technologies.
