Staff Pharmacist – Surgical Wards & Pre‑Admission Clinic

About the role
The Staff Pharmacist provides comprehensive pharmacy services to the inpatient surgical wards and the Surgical Pre‑Admission Clinic at Toronto General Hospital. This role is part of a multidisciplinary patient‑care team and includes cross‑coverage to other areas as needed.
Key responsibilities
- Conduct best possible medication history (BPMH) interviews and perform medication reconciliation.
- Review and dispense physicians' medication orders, ensuring appropriateness.
- Identify, resolve, and prevent drug‑therapy problems in collaboration with the health‑care team.
- Monitor drug therapy for effectiveness, toxicity, and adherence to evidence‑based guidelines.
- Follow up on medication incidents and promote safe medication practices.
- Respond to drug‑information queries, provide patient education, and facilitate hospital discharge.
- Supervise pharmacy technician staff directly or functionally.
- Support clinical teaching and research activities within the department.
Required profile
- Bachelor of Science in Pharmacy, Doctor of Pharmacy, or equivalent.
- Certificate of competency from the Pharmacy Examination Board of Canada.
- Active registration with the Ontario College of Pharmacists (Part A).
- Hospital pharmacy practice experience with a patient‑centered care focus.
- Experience in an inpatient acute‑care setting.
- Excellent verbal and written communication skills.
- Strong customer‑service orientation.
