Registered Practical Nurse – Long-Term Care

About the role

Join Extendicare’s inter‑professional team as a Registered Practical Nurse (RPN) delivering person‑centered care in a long‑term care setting. You will ensure resident assessments, care plans, and documentation are accurate, up‑to‑date, and communicated effectively.

Key responsibilities

• Complete and maintain resident clinical assessments, care/service plans, and related documentation.
• Implement, evaluate, and adjust care plans as needed.
• Assist with scheduling and participating in resident care conferences.
• Coordinate resident move‑ins, move‑outs, and transfers with DOC/DHW.
• Provide direct resident care, medication and treatment administration, and document compliance with regulatory standards.
• Process orders, safeguard drugs and narcotics, and administer them per policy.
• Deliver resident and family education.
• Plan, coordinate, manage, and evaluate resident care plans with interdisciplinary teams and service providers.
• Assign nursing team duties to ensure timely service delivery.
• Respond to significant changes in resident condition, medical emergencies, and provide first aid.
• Teach, delegate to, and direct unregulated care providers.
• Address resident/family concerns and communicate with leadership.
• Support infection prevention, health and safety, and quality/safety programs.

• Ability to work collaboratively within an inter‑professional team.
• Strong communication skills for resident, family, and staff interactions.
• Capacity to respond promptly to emergencies and changing resident conditions.
