Registered Practical Nurse – Acute Care (Mental Health)

About the role

We are seeking a Registered Practical Nurse (RPN) to join our acute‑care mental health team. The role focuses on delivering safe, evidence‑based, patient‑centred nursing care while collaborating with an inter‑professional team in a dynamic hospital environment.

Key responsibilities

• Provide holistic, culturally appropriate nursing care to stable patients, following College of Nurses of Ontario standards and Unity Health Toronto policies.
• Establish therapeutic relationships with patients and families, assess needs, and integrate them into care planning.
• Develop, implement and evaluate individualized care plans, using evidence‑based interventions.
• Monitor patient status, recognize rapid changes, and promptly consult Registered Nurses when instability arises.
• Deliver health education tailored to patient and family learning needs.
• Process and verify physician orders, document assessments and interventions accurately and timely.
• Coordinate with the care team to ensure seamless discharge and transition planning.
• Advocate for patients to receive care in the most appropriate setting.

Required profile

• Current registration as a Practical Nurse (RPN) in Ontario.
• Knowledge of professional standards set by the College of Nurses of Ontario.
• Ability to work autonomously with stable patients and collaboratively with RNs for unstable cases.
• Commitment to continuous learning, reflective practice and professional development.
• Strong communication skills to engage patients, families and the inter‑professional team.
