Registered Nurse / RPN – Child & Family Visiting Role (Toronto Central)

About the role

VHA Home HealthCare is seeking a full‑time Registered Nurse (RN) or Registered Practical Nurse (RPN) to join its Child & Family Program in Toronto Central. The visiting nurse will provide home‑based, client‑centred care to children and young adults up to 25 years of age with complex medical needs, supporting both health outcomes and family well‑being.

Key responsibilities

• Deliver nursing assessments, medication administration, and monitoring during home visits.
• Manage specialized care such as central line, G‑tube feeding, mechanical ventilation, tracheostomy, and ostomy.
• Provide seizure management, diabetes care, wound care, and catheterization.
• Support personal hygiene, diaper changes, and other daily living activities.
• Coordinate with families, physicians, and interdisciplinary team members to ensure continuity of care.

Required profile

• Current RN or RPN registration with the College of Nurses of Ontario.
• Valid Ontario driver’s licence (G2 or G) and reliable vehicle.
• Experience with central line, diabetes, wound, ostomy, and catheter care.
• Ability to work independently in community settings.
