Registered Nurse / Practical Nurse – Malton Clinic (Mississauga)

About the role

VHA Home HealthCare is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to join the Malton Clinic in Mississauga. The position supports an appointment‑based clinic that operates seven days a week, providing a wide range of home‑health services.

Key responsibilities

• Assess, monitor, and develop individualized care plans for clients.
• Evaluate care plan interventions through ongoing reassessment.
• Deliver wound care, medical injections, drain and ostomy care, chemotherapy connection/disconnection, and diabetes management.
• Ensure quality of patient care and act on factors affecting practice autonomy.
• Communicate changes in client condition or care plans to the nursing supervisor.

Required profile

• Current RN or RPN registration with the College of Nurses of Ontario.
• Availability to work Wednesdays, Fridays, and a bi‑weekly weekend rotation.
• Commitment to client‑centred, community‑based care.
