Registered Nurse – Hospital to Home (Full‑Time)

About the role

The Ontario Shores Centre for Mental Health Sciences is seeking a full‑time Registered Nurse to join its Hospital to Home (H2H) program. Reporting to the Clinical Manager for Complex Services, you will deliver specialised, recovery‑focused care to patients with severe and persistent mental illness as they transition from hospital to community settings.

Key responsibilities

• Provide direct nursing care and clinical assessment for complex mental health and addiction patients.
• Coordinate discharge planning, case management and community reintegration for long‑stay psychiatric inpatients.
• Collaborate with multidisciplinary teams, families, referral sources and community partners to ensure smooth, recovery‑oriented transitions.
• Document care accurately using Meditech and uphold standards of practice and relevant legislation.
• Act as a Recovery Champion, modelling the recovery philosophy in all interactions.
• Provide leadership, preceptorship and mentorship to nursing staff as needed.

Required profile

• Active registration with the College of Nurses of Ontario in good standing.
• Current CPR – Health Care Provider certification.
• Knowledge of CNO Standards of Practice, Canadian Psychiatric‑Mental Health Nursing standards, the Mental Health Act and PHIPA.
• Experience with community reintegration of long‑stay psychiatric patients.
• Strong interpersonal and communication skills, ability to work flexible daytime, evening and weekend shifts.
• BScN preferred; recent psychiatric nursing experience preferred.

Required skills

• CPR – Health Care Provider certification.
• Proficiency with Meditech electronic health record system.
• General computer literacy for documentation and communication.
