Registered Nurse – Home & Community Care

About the role

SE Health is seeking a compassionate Registered Nurse to join our supportive team in London, Ontario. You will provide comprehensive, client‑focused care in home and community settings, working evenings on a full‑time basis.

Key responsibilities

• Conduct thorough client assessments and develop individualized care plans.
• Implement, monitor, and evaluate care based on best evidence and client needs.
• Serve as primary provider for a caseload, coordinating with multidisciplinary team members.
• Facilitate care transitions and connect clients with local community support services.

Required profile

• Current Certificate of Competence with the College of Nurses of Ontario (RN).
• Availability to work full‑time evenings (3 p.m. – 11 p.m.).
• Up‑to‑date immunization records, clear criminal background check, and vulnerable sector check.
• Willingness to obtain SE Health certifications and maintain a current CPR certificate.
• Physically able to sit, stand, walk, bend, kneel, reach, and perform hand‑grip tasks; travel to London and surrounding areas.

Required skills

• RN certification (College of Nurses of Ontario).
• CPR certification.
