About the role
SE Health is seeking a compassionate Registered Nurse to provide high‑quality, community‑based care across Ontario. You will join a supportive team that values flexible scheduling, ongoing education, and a strong work‑life balance.
Key responsibilities
- Conduct comprehensive client assessments and develop individualized care plans.
- Implement and evaluate care based on best‑evidence practices and client needs.
- Serve as primary provider for a caseload, coordinating with multidisciplinary team members.
- Facilitate care transitions by linking clients with local community support services.
Required profile
- Current registration with the College of Nurses of Ontario.
- Current CPR certification.
- Valid driver’s licence and personal vehicle for travel between client sites.
- Up‑to‑date immunisation records, clear criminal background check, and vulnerable sector check.
- Willingness to obtain Saint Elizabeth certifications.
- Availability to work days and every other weekend.
- Physical ability to lift up to 55 lb, climb stairs, and perform repetitive hand‑arm movements.
