Registered Nurse – Community Care

About the role

SE Health is seeking a compassionate Registered Nurse to provide high‑quality, community‑based care across Ontario. You will join a supportive team that values flexible scheduling, ongoing education, and a strong work‑life balance.

Key responsibilities

• Conduct comprehensive client assessments and develop individualized care plans.
• Implement and evaluate care based on best‑evidence practices and client needs.
• Serve as primary provider for a caseload, coordinating with multidisciplinary team members.
• Facilitate care transitions by linking clients with local community support services.

Required profile

• Current registration with the College of Nurses of Ontario.
• Current CPR certification.
• Valid driver’s licence and personal vehicle for travel between client sites.
• Up‑to‑date immunisation records, clear criminal background check, and vulnerable sector check.
• Willingness to obtain Saint Elizabeth certifications.
• Availability to work days and every other weekend.
• Physical ability to lift up to 55 lb, climb stairs, and perform repetitive hand‑arm movements.
